Note for South Asian populations: Many health bodies recommend lower BMI cutoffs for South Asians (including Indians) due to higher body fat percentage and diabetes/heart disease risk at lower BMI levels โ€” often 23 for "overweight" and 27.5 for "obese" instead of the standard 25 and 30.

Where BMI Falls Short

BMI was designed in the 1830s by a Belgian mathematician for population-level statistics โ€” not individual diagnosis. Its major limitations:

  • Doesn't distinguish muscle from fat: A muscular athlete can have a "obese" BMI while having very low body fat
  • Doesn't account for fat distribution: Belly fat (visceral) is riskier than hip/thigh fat, but BMI treats all weight the same

  • Can misclassify elderly people who've lost muscle mass (sarcopenia) as "normal" while actually being at risk
